A strong answer

Hold the third dose. Nitroglycerin already drops blood pressure by dilating vessels โ€” his own hypotension is now stacked on top of that effect. Next: recheck the BP, keep him supine (legs elevated if he tolerates it), and notify the prescriber before giving anything else for the pain. Pushing another dose right now could drop his pressure further and reduce coronary perfusion โ€” the opposite of what you're trying to do for a heart that's already struggling.
